RELEASE
By clicking below and intending to be bound, and acting for myself and for others I may invite to the 2011 Kumon Instructors Conference as my guest, I hereby release and forever discharge KUMON NORTH AMERICA, INC., KUMON CANADA, INC. and KUMON MEXICO, INC. (collectively, “Kumon”), their agents, servants and employees, for myself and my heirs, executors, administrators and assigns, from any and all claims, demands, rights and causes of action whatsoever of whatever kind and nature, arising from and by reason of any and all known and unknown, foreseen and unforeseen, bodily and personal injuries, damage to property and the consequence thereof , that result or may result from my travel to and from, and my attendance at the 2011 Kumon Instructors Conference.
The reimbursement of expenses may differ from any estimate given. Kumon has disclosed to me its policy on reimbursement for travel and other cost I may incur by reason of my attendance (and the attendance of my invited guests) at the 2011 Kumon Instructors Conference. I understand that any estimated reimbursement is based on information I submitted and is subject to actual expense verification and recalculation based on Kumon’s reimbursement policy. My actual reimbursement shall be governed by the application of Kumon’s policy to the actual expenditures incurred by me. |
